Pneumococcal vaccine, polyvalent
Indications/Uses
Vaccination against pneumococcal disease caused by 23 pneumococcal serotypes covered by Pneumovax 23 for persons ≥50 yr.
Dosage/Direction for Use
SC/IM Single 0.5-mL dose (preferably in the deltoid muscle or lateral mid-thigh). Revaccination: All persons ≥65 yr who have not received vaccine w/in 5 yr (& were <65 yr at the time of vaccination) should receive another dose of vaccine.
Contraindications
Hypersensitivity. Epinephrine inj (1:1,000) must be immediately available should an acute anaphylactoid reaction occur.
Special Precautions
Do not inj IV or intradermally. Ensure at least 2 wk interval between Pneumovax 23 administration & initiation of cancer chemotherapy or other immunosuppressive therapy (eg, patients w/ Hodgkin's disease or undergoing organ or bone marrow transplantation), & before elective splenectomy. Pneumococcal vaccine may be given several mth following completion of chemotherapy or RT for neoplastic disease. Revaccination following a 2nd dose is not routinely recommended. Not to be given concomitantly w/ Zostavax. In persons receiving immunosuppressive therapy, expected serum Ab response may not be obtained & may impair future immune response to pneumococcal antigens. Individuals w/ severely compromised CV &/or pulmonary function. Delay usage in febrile resp illness or other active infection. Patients requiring penicillin (or other antibiotic) prophylaxis against pneumococcal infection, such prophylaxis should not be discontinued after vaccination w/ Pneumovax 23. May not result in complete protection in all recipients. Pregnancy & lactation. Frail elderly individuals w/ multiple co-morbid conditions.
Adverse Reactions
Fever (≤38.8°C), inj site reactions (eg, pain, soreness, erythema, warmth, swelling, local induration), decreased limb mobility & peripheral edema in the injected extremity. Asthenia/fatigue, myalgia & headache.
